That 2025 release for Pathologic 3 that HypeTrain Digital and Ice-Pick Lodge were aiming for is out of reach, but you won’t have to wait long. The teams have announced that the game will see release on January 9, 2026 on Steam, with PlayStation 5 and Xbox Series versions to follow at a later date. In addition, you can check out Pathologic 3 for yourself with the demo during Steam Next Fest.
In Pathologic 3, players once again find themselves in a small, remote town gripped by a mysterious and deadly disease. You play as Daniil Dankovsky, a metropolitan physician who arrives to meet an “immortal” man, but is forced to lead the fight against the epidemic and try to save the city in just 12 days. As a result, you must make ruthless decisions and diagnose with precision.
That involves diagnosing and treating patients based on examinations. You’ll be using a microscope and research pathogens to develop treatments and save your patients. Fight the epidemic by implementing quarantines and deploying patrols to lock down the spread of disease.
New to the series is a time-travel mechanic that allows players to revisit key events and alter them to their advantage. While you still only have 12 days to save the town, this mechanic allows you the freedom to explore and influence events in ways that reshape the unfolding narrative.
The Steam Next Fest demo offers players an introduction to the full game, with entirely unique content that hasn’t been shown anywhere before.
